在新闻中，往往提到一些血腥、恐怖或者赤裸裸的性有关的内容或画面时，往往会提示viewer：the following contains graphic images or videos，但奇怪的是，很少有非ESL词典单独提出来这个意项，有的在描写生动的这种意项下给出了几个例子倒还勉强可以理解出这个意思，有的干脆例子也不给，在生动的下面给出的是诸如graphic account这样的解释，这根本不足以理解到画面、视频或描述可能会带来不适unpleasant这样的含义，相反，几部高阶词典均给出了unpleasant这样的含义，我比较好奇的是如果一个English native speaker不看高阶能从那些母语使用者词典诸如merriam collegiate中理解到那种unpleasant的含义吗？这个用法在新闻中使用得非常非常频繁，几乎一到这种时候就会有这个graphic的提示，我第一次遇到大概是ISIS对人质进行beheading的那个新闻

如你所言，它原来是和 account 等搭配的，不见得是 unpleasant 的。ESL 字典收的应该算衍生义，但是母语词典可能保守一些，没有认为必须把这义项独立出来。
